Hooyo Home Health, LLC provides Structured Community Day Services designed to support individuals with developmental disabilities in building daily living, social, and functional skills. Our program promotes independence, community participation, and meaningful engagement in a safe and supportive environment. Services are individualized based on each person’s goals, strengths, and Personal Plan.

Program Overview

 

Hooyo Home Health, LLC provides Structured Community Day Services designed to support individuals with developmental disabilities in developing daily living, social, and functional skills. Our program promotes independence, confidence, and meaningful participation in the community. Services are individualized and based on each person's Personal Plan, goals, and strengths, ensuring that every individual receives support tailored to their needs.

Skill Development and Daily Living

Our program focuses on helping individuals build essential life skills that promote independence and personal growth, including:Daily living skills Social and communication skills Personal responsibility Self-advocacy and decision making Community participation​Activities are structured to support learning, routine, and positive social interaction.

Community Integration

 

Community participation is an important part of our program. Individuals are supported in engaging with their community through supervised activities that encourage social interaction, confidence, and meaningful experiences.

Examples include:

  1. Community outings

  2. Recreational activities

  3. Educational and enrichment activities

  4. Volunteer opportunities

Person-Centered Support Goal

 

Hooyo Home Health delivers services using a person-centered approach, respecting each individual's preferences, abilities, and goals. We work closely with families, guardians, and Independent Service Coordination (ISC) agencies to ensure coordinated and supportive care.

Program Compliance - Illinois Rule 119

Hooyo Home Health, LLC operates in compliance with the Illinois Department of Human Services (IDHS) Rule 119 regulations governing Community Day Services. Our program maintains compliance by ensuring:

  1. Services are based on each individual's Personal Plan

  2. Individuals receive person-centered supports that promote independence and choice

  3. Staff receive required training and qualifications

  4. Programs promote community integration and skill development

  5. Health, safety, and dignity of individuals are protected at all times​

Referrals and Enrollment

Individuals interested in Community Day Services may be referred through their Independent Service Coordination (ISC) agency, family members, or guardians.
