Our Mission
Hooyo Home Health, LLC is a licensed provider of Community Day Services training programs dedicated to empowering individuals with developmental disabilities. Our program focuses on promoting independence, skill development, community integration and overall well being. At Hooyo Home Health, we recognize that every individual has unique strengths, abilities, and goals. That's why we create personalized service plans designed to meet each participant's specific needs and preferences. These plans may include life skills training, communication support, social development, vocational readiness, and community engagement activities.

Individual Support &Skill Development
Hooyo Home Health provides individualized support services designed to help individuals develop daily living, social, and functional skills. Services are tailored to each individual's Personal Plan and focus on promoting independence, confidence, and meaningful participation in daily activities.
Structured Community Day Services
Hooyo Home Health offers structured Community Day Services in a safe, supportive, and inclusive environment. Programs are designed to provide routine, skill building activities, and opportunities for social interaction that enhance overall quality of life.
Family Support and Collaboration
We work collaboratively with families to ensure coordinated, person-centered care. Family support services include guidance, communication, and education to strengthen partnerships and promote positive for individuals receiving services
FamilCommunity Integration & Social Engagementy Support and Collaboration
We support individuals in building social skill and engaging meaningfully within their community. Services promote inclusion, relationship-building, and participation in community-based activities that support independence and personal fulfillment.
Personalized Developmental Training
Our developmental training programs are individualized to meet each individual's goals, strengths, and support needs. Training focuses on life skills, self-advocacy, communication, and community, and delivered at a pace that support learning and growth.
Person-Centered Plan
Our programs incorporate therapeutic and wellness-focused activities that support emotional well-being, self-expression, and personal development. All services are delivered using a person-centered approach that respects individual choice, dignity, and cultural values.
Educational Training approved by Illinois department of health and human services
QIDP 40 hour training is a state-required foundational course for Qualified Intellectual Disability Professional in illinois. It provides essential instruction on person-centered planning, program development, documentation, compliance, and service coordination to ensure individuals with developmental disabilities receive safe, effective, and compliant supports.
Direct Support Professional (DSP) provides hands-on support to individuals with developmental disabilities, assisting with daily living skills, community participation, safety, and independence while following person-centered plans and agency policies.
